The ideal candidate will work closely with licensed physical therapists to provide high-quality patient care in a skilled nursing facility. This role is crucial in helping patients regain mobility and improve their quality of life through effective rehabilitation strategies. Duties Assist physical therapists in implementing treatment plans Conduct patient assessments and monitor progress to ensure effective treatment outcomes. Support patients in performing therapeutic exercises and activities designed to improve strength, flexibility, and mobility. Educate patients and their families about treatment plans, care plans, and exercises to be performed at home. Document patient care activities accurately and maintain comprehensive records of treatments provided. Collaborate with the healthcare team to develop discharge planning strategies that facilitate smooth transitions for patients. Maintain a clean and safe working environment by adhering to health and safety protocols. Requirements Associate degree in Physical Therapy Assisting or a related field from an accredited program. Current state licensure or certification as a Physical Therapist Assistant. Experience in physical therapy settings such as outpatient clinics, skilled nursing facilities, or home health is preferred. Knowledge of patient care techniques and rehabilitation practices relevant to various populations Strong communication skills to effectively interact with patients, families, and healthcare team members. Ability to work collaboratively within a multidisciplinary team while demonstrating compassion and empathy towards patients. Familiarity with discharge planning processes and the development of individualized care plans is a plus.
