Duties & Responsibilities Provide rehabilitative and skilled PT services to patients in their homes under the supervision of a licensed Physical Therapist Perform manual therapy and functional exercises with patients Report changes in patient condition to the supervising PT and interdisciplinary team Assist in the supervision of CNAs Utilize various assessments to identify patient needs and adjust the plan of care under PT direction Complete accurate and timely point-of-care documentation Meet agency productivity expectations Job Requirements Graduate of an accredited Physical Therapy Assistant program Current Oregon PTA license Valid Oregon driver's license Valid auto insurance Current CPR certification Home health experience preferred Our Mission We believe in a holistic approach to care—valuing each person's physical, emotional, and spiritual needs.
