This position offers flexible, as-needed scheduling while providing one-on-one care to patients in their homes.

Key Responsibilities:

Provide in-home physical therapy services under the supervision of a licensed Physical Therapist Assist in implementing individualized treatment plans to improve patient mobility, strength, and function Monitor patient progress and report changes to the supervising Physical Therapist Educate patients and caregivers on prescribed exercises, safety techniques, and home care routines Maintain accurate and timely documentation in accordance with company policies and regulatory standards Communicate effectively with the care team to ensure coordinated and high-quality patient care Travel to patient homes within the assigned service area

Qualifications:

Graduate of an accredited Physical Therapist Assistant program Active Physical Therapist Assistant license in the state of Tennessee Current CPR certification Valid driver's license, reliable transportation, and current auto insurance Strong interpersonal and communication skills Ability to work independently and manage time efficiently Previous home health experience preferred but not required
