Physical Therapist (PT) & Physical Therapy Assistant (PTA) - Full Time Dallas, TX | The Legacy at Midtown Park The Legacy at Midtown Park is currently seeking compassionate and motivated Physical Therapists (PTs) and Physical Therapy Assistants (PTAs) to join our exceptional in-house rehabilitation team in our skilled nursing facility in Dallas, Texas. Our therapy professionals are dedicated to delivering person-centered care that helps residents achieve meaningful outcomes, improve mobility, and maximize independence. Position Summary The Physical Therapist and Physical Therapy Assistant will work collaboratively to provide skilled rehabilitation services designed to improve mobility, strength, balance, endurance, and overall functional independence for residents in a skilled nursing setting. Responsibilities Evaluate and treat residents with physical limitations, mobility impairments, pain, and functional deficits Develop and implement individualized treatment plans in accordance with physician orders and established plans of care Provide skilled therapy interventions to improve strength, balance, gait, endurance, and overall mobility Monitor patient response to treatment and document progress toward functional goals Communicate patient progress, concerns, and recommendations with the interdisciplinary care team Educate patients, family members, and caregivers regarding exercises, mobility, safety, and treatment plans Maintain accurate, timely, and compliant documentation according to company policy and state/federal regulations Collaborate with nursing staff and healthcare professionals to ensure quality patient outcomes Attend required meetings and participate in care planning as directed by the Director of Rehabilitation Qualifications For Physical Therapist (PT): Active and valid Texas Physical Therapist license in good standing Graduate of an accredited Physical Therapy program For Physical Therapy Assistant (PTA): Active and valid Texas Physical Therapy Assistant license in good standing Graduate of an accredited Physical Therapist Assistant program Preferred Qualifications Skilled nursing facility (SNF) or long-term care experience preferred Knowledge of Net Health electronic documentation system preferred Strong communication and interpersonal skills Ability to work effectively with a diverse patient population Career Advantages In-house rehabilitation team (non-contracted) Competitive compensation and benefits Flexible scheduling Supportive and collaborative team environment Professional growth opportunities New graduates encouraged to apply Benefits (Full-Time Only) Medical Insurance Dental Insurance Vision Insurance Life Insurance Paid Time Off Retirement Plan Join a team that values compassionate care, clinical excellence, and professional development.
