In this dynamic role, you will work closely with licensed physical therapists to deliver high-quality, personalized care to patients in their homes. Your expertise will help improve mobility, reduce pain, and promote independence for diverse patient populations, including geriatrics, pediatrics, and individuals recovering from surgery or injury. This position offers an exciting opportunity to make a meaningful difference in patients' lives while expanding your skills in physiotherapy and patient care. Duties Assist licensed physical therapists in developing and implementing individualized care plans tailored to each patient's needs. Provide hands-on therapies such as trigger point therapy, therapeutic exercises, and manual techniques to improve mobility and reduce discomfort. Conduct patient assessments, monitor progress, and document treatment outcomes accurately. Educate patients and their families on physiotherapy techniques, home exercises, and injury prevention strategies. Manage discharge planning by coordinating follow-up care and ensuring seamless transitions between care settings. Maintain detailed records of treatments administered, patient responses, and any changes in condition. Collaborate with healthcare team members to ensure comprehensive patient care across inpatient, outpatient, and home health environments. Qualifications Valid licensure or certification as a Physical Therapist Assistant (PTA) in accordance with state regulations. Proven experience working in home health settings; inpatient or skilled nursing facility experience is highly desirable. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques. Familiarity with outpatient clinics, orthopedics, geriatrics, pediatrics, and acute care environments. Skilled in developing and implementing effective care plans tailored to diverse patient populations. Experience with discharge planning processes and collaborating within multidisciplinary teams. Proficiency in using therapeutic modalities such as trigger point therapy and other manual therapy techniques.
