Job Description
Job Overview Join our dedicated healthcare team as a Home Health Physical Therapist Assistant, where you will play a vital role in delivering exceptional patient care in the comfort of patients' homes. This dynamic position offers the opportunity to work closely with licensed physical therapists to implement personalized care plans, promote recovery, and enhance quality of life for diverse patient populations. Your expertise will directly impact patients' mobility, independence, and overall well-being, making a meaningful difference every day. Duties Assist licensed physical therapists in developing and executing individualized treatment plans tailored to each patient's needs. Provide hands-on therapy interventions such as therapeutic exercises, manual therapy techniques including trigger point therapy, and functional training to improve mobility and reduce pain. Monitor and document patient progress, adjusting care approaches as necessary to optimize outcomes. Educate patients and their families on proper exercise techniques, safety measures, and injury prevention strategies. Support discharge planning by preparing patients for continued care at home or transition to outpatient or skilled nursing facilities. Maintain accurate and timely documentation of treatments, assessments, and patient responses in compliance with healthcare regulations. Collaborate with interdisciplinary teams to ensure seamless coordination of care across inpatient, outpatient, home health, and skilled nursing settings. Requirements Current certification as a Physical Therapist Assistant (PTA) with valid licensure or registration in the relevant state. Proven experience working in home health environments, skilled nursing facilities, or outpatient clinics. Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practices. Hands-on experience with physiotherapy techniques including trigger point therapy and orthopedic rehabilitation. Familiarity with discharge planning processes and managing care for geriatric, pediatric, and adult populations. Ability to perform effectively in both inpatient and outpatient settings while demonstrating excellent patient care skills.
