Job Description

Job Overview We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. In this role, you will be responsible for evaluating, developing, and implementing personalized treatment plans to improve patients' mobility, strength, and overall well-being. The ideal candidate will possess a comprehensive understanding of human physiology and anatomy, along with experience in various care settings including inpatient, outpatient, home health, and hospital environments. Your expertise will support patients across diverse age groups and medical conditions, ensuring high-quality patient care through evidence-based physiotherapy practices. Duties Conduct thorough assessments of patients' physical conditions, including mobility limitations and functional impairments. Develop individualized care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and other modalities to promote recovery. Implement treatment interventions aligned with best practices in orthopedics, geriatrics, pediatrics, and rehabilitation. Monitor patient progress regularly and adjust care plans accordingly to optimize outcomes. Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transition of care within hospital or community settings. Document patient evaluations, treatment sessions, and progress notes accurately using electronic medical record (EMR) systems. Educate patients and their families on physiotherapy exercises, injury prevention strategies, and home care routines. Assist in training new staff or interns by sharing expertise in anatomy knowledge, physiology principles, and clinical techniques. Support outpatient clinics or home & community care programs by providing expert physiotherapy services tailored to individual needs. Experience Proven experience working in inpatient hospital settings with a strong understanding of acute care protocols. Demonstrated proficiency in outpatient clinics or home health environments with a focus on patient-centered care. Knowledge of medical terminology relevant to physiotherapy and rehabilitation medicine. Hands-on experience with discharge planning processes for diverse patient populations. Familiarity with EMR systems for efficient documentation and communication. Background in pediatrics, geriatrics, orthopedics, or sports medicine is highly desirable. Prior involvement in home & community care programs that require adaptability across various environments. Strong understanding of physiology and anatomy to inform effective treatment strategies. Experience utilizing manual therapy techniques such as trigger point therapy to alleviate pain and improve function.
