Job Description

Job Summary Join our dynamic healthcare team as a Physical Therapy Assistant in a Skilled Nursing Facility (SNF), where you will play a vital role in helping patients regain their strength, mobility, and independence. In this energetic and rewarding position, you will collaborate closely with licensed physical therapists to deliver high-quality patient care across inpatient settings. Your expertise in physiotherapy techniques, anatomy, physiology, and medical terminology will directly impact patients' recovery journeys. Whether working with geriatric populations, pediatric cases, or those recovering from orthopedic or acute care events, your dedication will make a meaningful difference every day. Duties Assist licensed physical therapists in developing and implementing individualized care plans tailored to each patient's needs. Provide hands-on therapy interventions such as trigger point therapy, therapeutic exercises, and mobility training to promote recovery. Monitor patient progress, document responses to treatments, and communicate updates to the healthcare team. Support discharge planning by preparing patients for transition back home or to other care settings through education and functional training. Maintain accurate medical records, treatment logs, and progress notes in compliance with facility policies. Educate patients and their families on physiotherapy techniques, safety measures, and post-discharge exercises. Ensure a safe environment by adhering to infection control protocols and assisting with patient transfers or positioning. Experience Demonstrated knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practices. Hands-on experience with trigger point therapy and other manual therapy techniques is advantageous. Familiarity with discharge planning processes and coordination of care for diverse patient populations including geriatrics, pediatrics, orthopedics, and those recovering from acute care procedures. Strong understanding of care plans for patients in outpatient or clinic environments related to orthopedics or geriatrics. Experience working with inpatient populations requiring skilled nursing services enhances your ability to deliver comprehensive patient care effectively.
