Physical Therapy Assistant - Home Health

Job Summary Join our dedicated healthcare team as a Physical Therapy Assistant specializing in home healthcare! The primary service region would include: Eau Claire, Black River Falls, Osseo, and surrounding communities with an agency provided fleet vehicle . In this dynamic role, you will provide essential rehabilitative services directly in patients' homes, helping individuals regain mobility, reduce pain, and improve their overall quality of life. Your expertise will support personalized care plans, discharge planning, and coordination with multidisciplinary teams to ensure seamless recovery journeys. Responsibilities Deliver skilled physical therapy interventions tailored to each patient's needs, including exercises, manual therapy techniques, balance and mobility training. Assist with developing and implementing comprehensive care plans in collaboration with licensed therapists and nurses. Monitor patient progress regularly, adjusting treatment approaches to optimize outcomes in collaboration with supervising physical therapist. Educate patients, their families, and caregivers on therapy techniques, home exercises, and injury and fall prevention strategies. Document all patient interactions accurately in medical records, including treatment details and progress notes. Support discharge planning by preparing patients for continued care at home or transition to other healthcare services. Maintain a safe environment by adhering to infection control protocols and safety guidelines during all patient interactions. Experience Prior experience working inpatient settings such as skilled nursing facilities, acute care units, or home health is highly preferred but not required. Knowledge of anatomy, physiology, medical terminology, and orthopedic rehabilitation techniques is essential. Familiarity with older adult and geriatric populations. $25.00 - $35.00 per hour
