Job Description

Crescent City Care Center is seeking a Physician Assistant to support a busy, patient-centered skilled nursing facility . You will manage and coordinate care in collaboration with the supervising physician. What you'll do Manage acute and chronic conditions. Order, interpret, and evaluate diagnostic tests. Educate patients and families on conditions, self-care, medications, and adherence. Participate in weekly interdisciplinary care plan meetings. Collaborate with physicians on complex cases. Prescribe medications and treatments as appropriate. Provide care across settings, including telehealth, in-person visits, and field/home visits. Maintain privileges in multiple nursing homes. Document visits electronically and support quality improvement initiatives. Review, approve, and modify admission orders. Complete admission notes within 24 hours, including medication reconciliation. Initiate and document Advanced Directives and POLST. Confirm and act on Health Care Proxy status when appropriate. Partner with case management and rehab on long-term care evaluations and discharge planning. Follow long-term patients at least every 30 days. Create discharge summaries with medication reconciliation and send to the PCP at discharge. Qualifications Current, unrestricted California PA license (required). Current NCCPA certification (required). 2+ years of clinical experience preferred, including skilled nursing facility or post-acute care experience. Strong communication skills and ability to build rapport. Comfortable in a fast-paced environment. Current state driver's license. Knowledge of CPT, ICD-10, and HCPCS codes. Knowledge of clinical standards of care. Familiarity with UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare benefits (helpful). Passion for geriatric care and comfort with home-based visits. Education & licensure Graduation from an accredited Physician Assistant program (required). Active PA license in California (required). Prescribing authority and DEA registration in good standing (preferred, as applicable).
Pay:
$80.00 - $110.00 per hour
