Job Description
Bluerock Care provides primary care without walls to mid- and lower-income adults on Medicare in mostly underserved areas. We combine virtual, home, and office-based care to meet people's needs where and when they arise. We are a value-based provider:
through our contracts with payers, we aim to be rewarded for quality and outcomes, not volume. We also believe that practitioners should be able to spend time with patients and the care team -- not overly burdened by administrative tasks. We are located at 3230 Pennsylvania Ave, SE, Washington DC 20020, and we serve patients in both DC and MD. With our unique care model, we aim to grow rapidly both regionally and nationally. Essential job duties and responsibilities We are looking for a PA to join our team. The responsibilities will be, amongst others: Practice primary care through office visits, virtual visits, and home visits in a way that sets a high standard for other clinicians joining our primary care team. Work collaboratively with our care team (Registered Nurses, Medical Assistants, Clinical Assistants, among others) to collectively manage patients' care. Appropriately order diagnostic studies, interpret results, and create individualized treatment plans using evidence based medicine. Manage medical and surgical conditions with knowledge of indications, contraindications, and adverse effects of treatment modalities. Counsel and educate patients and families. Perform accurate and timely documentation in the electronic medical record. Be a role model, mentor and coach for all others on the team. The ideal candidate has a strong affinity or connection with the populations we serve.Key criteria include:
Leadership skills, passion for teamwork, ability to coach/mentor team members in a young, dynamic organization. Unrestricted license to practice as a PA in DC and Maryland. CDS and DEA for both DC and Maryland preferred Minimum of 1 year work experience as a PA. Relevant experience (working with elderly patients, lower-income patients and/or palliative care) preferred Enthusiastic to work in a performance-driven organization (data-driven, focused on continuous improvement of patient experience, care quality, and outcomes) Experience with Electronic Medical Records and office software.
