Job Description

Physician Assistant - Certified 85 Seasons Ln, Hiawassee, GA 30546 Full-time, Contract Full-time, Contract Synergy Health, Inc. is seeking a full-time Physician Assistant (PA) to deliver excellent patient care in Hiawassee, GA. The PA/NP will provide preventive and sick care in a busy family practice, maintain accurate clinical records in our EHR system, and utilize Accountable Care Organization (ACO) tools to assist patients in accessing high value healthcare.
Responsibilities:
l Provide office-based, clinical primary care including screenings, history and physical, care plan documentation, vaccination and high-value referrals. l Enhance delivery of well-coordinated care by incorporating records from acute, specialist and post-acute visits into the clinical chart and deriving clinical insights. l Deliver high-quality care and document appropriately to maintain excellent quality scores within commercial and public payer ACO programs. l Provide efficient care to minimize patient wait time and achieve practice productivity goals. l Maximize patient satisfaction by providing evidence-based patient education, shared decision making and excellent bedside manner.
Qualifications:
l Graduation from an accredited program and current licensure to practice in the state of Georgia. l Experience in a primary care or comparable care setting preferred. l Experience documenting clinical data in an electronic health record system (EHR). l Strong written and interpersonal communication skills. l Team-oriented with a positive attitude. l Philosophically aligned to ACO goals of high quality and high value care.
