This a full-time position providing the opportunity to care for patients ages 14 and older in a fast-paced, professional environment. The ideal candidate is confident evaluating, diagnosing, and treating a wide range of acute and chronic conditions while delivering high-quality patient care. We are looking for a provider who values patient relationships, communication, and teamwork and is passionate about helping patients achieve better health and quality of life. Responsibilities Provide comprehensive primary care services for patients ages 14 and older, including evaluation, diagnosis, treatment, and follow-up care. Perform preventive, wellness, chronic disease management, and acute sick visit assessments. Develop and manage individualized treatment plans based on current clinical guidelines and patient needs. Order, review, and interpret laboratory tests, imaging studies, and other diagnostic procedures. Communicate diagnoses, test results, treatment recommendations, and follow-up plans in a clear and compassionate manner. Educate patients and families on medical conditions, medications, preventive care, and healthy lifestyle choices. Coordinate patient care through referrals to specialists and community resources when appropriate. Monitor and manage chronic health conditions to support continuity of care and long-term patient wellness. Maintain accurate, complete, and timely documentation within the electronic health record (EHR). Collaborate closely with physicians, advanced practice providers, nursing staff, medical assistants, and administrative personnel. Contribute to practice initiatives focused on improving patient care, operational efficiency, and patient satisfaction. Foster a professional, welcoming, and patient-focused environment that reflects the practice's commitment to high-quality care.
Pay:
$140,000.00 - $160,000.00 per year
