Position Summary:

This is a career-building opportunity for a Physician Assistant (PA-C) seeking a better balance between professional impact and personal life. In this role, you will provide on-site rounding care , manage chronic conditions, and build meaningful relationships with patients and care teams. Responsibilities:

Provide on-site rounding care in assisted living, memory care, and long-term care communities . Perform assessments, chronic disease management, and preventative care . Develop and adjust patient-centered care plans . Collaborate with physicians, nurses, and interdisciplinary teams . Communicate with families, caregivers, and facility staff . Reduce hospitalizations through proactive care delivery . Complete documentation, medication management, and lab coordination .

Qualifications:

Active Minnesota Physician Assistant (PA-C) license in good standing . Experience in geriatrics, family medicine, or post-acute care preferred . Strong clinical and communication skills . Valid driver's license and reliable transportation. Passion for relationship-based senior care . Military Occupational Classification (MOC)
