We're seeking a Physician Assistant to provide primary care services within assisted living communities, memory care settings, and senior housing campuses throughout Western Minnesota. Rather than waiting for patients to navigate the healthcare system, you'll bring care directly to them, helping improve access, continuity, and outcomes for aging adults. As healthcare continues shifting toward community-based care, providers have an opportunity to play a larger role than ever before. You'll become a familiar face within your communities, partnering with nurses, caregivers, families, and leadership teams to help patients remain healthier, safer, and more independent. What You'll Be Doing Provide primary care services directly within assisted living, memory care, and senior living communities. Manage acute and chronic medical conditions while supporting preventative care initiatives. Collaborate closely with physicians, nurse practitioners, nursing teams, therapists, and caregivers. Help reduce unnecessary hospitalizations through proactive clinical intervention. Build strong relationships with patients, families, and interdisciplinary care teams. Qualifications Graduate of an accredited Physician Assistant program. Active Minnesota PA-C license required. Experience in family practice, internal medicine, urgent care, hospital medicine, geriatrics, post-acute care, or related settings preferred. Strong clinical judgment and confidence caring for medically complex patients.
