What You'll Be Doing Provide primary care services directly within assisted living, memory care, and senior living communities. Manage acute and chronic medical conditions while supporting preventative care initiatives. Collaborate closely with physicians, nurse practitioners, nursing teams, therapists, and caregivers. Help reduce unnecessary hospitalizations through proactive clinical intervention. Build strong relationships with patients, families, and interdisciplinary care teams. Qualifications Graduate of an accredited Physician Assistant program. Active Minnesota PA-C license required. Experience in family practice, internal medicine, urgent care, hospital medicine, geriatrics, post-acute care, or related settings preferred. Strong clinical judgment and confidence caring for medically complex patients.
