As a Wound Care & Geriatric Rounding Provider, you'll step into a highly specialized role that combines advanced wound management, geriatric medicine, chronic disease management, and community-based care . Responsibilities Provide on-site rounding, primary care, and advanced wound care services. Manage pressure injuries, diabetic ulcers, surgical wounds, skin integrity concerns, and complex chronic conditions. Perform assessments, diagnostics, treatment planning, and follow-up care. Drive chronic disease management and preventative care initiatives. Collaborate with physicians, nurses, therapists, caregivers, and facility staff. Help reduce avoidable hospitalizations through proactive clinical intervention and continuity of care. Complete documentation, medication management, and lab coordination. Qualifications Active Minnesota Physician Assistant ( PA-C ) license in good standing. Experience in wound care, geriatrics, family medicine, internal medicine, primary care, or post-acute settings preferred. Strong clinical judgment and confidence managing medically complex patients. Valid driver's license and reliable transportation. Passion for delivering outcomes-driven, patient-centered senior care.
