Additional Information About the Role Nurse Practitioner or Physician Assistant for Cardiology - Heart Failure Program. Monday -
Friday Days No Weekends No BJC Observed Holidays Heart Failure Program:
Provide medical services to patients with chronic heart failure to reduce hospital readmissions, and improve patient quality of life through personalized care, medication management and lifestyle education. Will see patients in outpatient clinics at St. Peters, Progress West and Wentzville. For questions, and/or further details, please reach out to julia.parker2bjc. Preferred Qualifications Role Purpose Utilizes a high degree of kledge, skill andpetence in a specialized patient care area. Assesses, diagnoses, prescribes, initiates plan of care and evaluates oues within the scope of a formal collaborative practice agreement with physician(s). Responsibilities Manages the health care of patient populations withpassion through advanced health assessment, diagnosis, intervention, and evaluation in a collaborative practice agreement with a physician. Prescriptive responsibilities are reviewed by collaborative physician. Coordinates the care of patient populations, in a collaborative practice agreement with a physician, to ensure promotion of health and wellness through education of patients and families, coordination of services and placing referrals. Develops treatment plans, treats patients with acute problems including illnesses and minor injuries following a predetermined protocol in a collaborative practice agreement with a physician. BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job. Minimum Requirements Education Master's Degree - Nursing Experience
