Advanced Clinical Provider- Hospital at Home Program At NewYork-Presbyterian, our Physician Assistants are more than skilled clinicians-they are trusted collaborators and vital members of our care teams.
Key Responsibilities:
Provide clinical care to HAH patients under the direction of the Hospital at Home Medical Directors, Supervising Physician, and Associate Chief Advanced Clinical Provider Deliver high-quality clinical care to HAH patients across emergency departments, inpatient units, and the Virtual Unit. Manage the full continuum of care for HAH patients, including enrollment, admission, daily clinical evaluations, discharge, consultative services, clinical monitoring, and telehealth visits Demonstrate an ability to make appropriate, highly complex clinical decisions and take effective action Perform on-going assessment of therapy and modify patient management plan as necessary Collaborate with interdisciplinary team members to ensure continuity of care and optimize the use of hospital resources. Assists the physician as directed in the training of health personnel in diagnostic, therapeutic and clinical techniques. Participate in continuing education and professional development activities to maintain clinical excellence. •Shift:
This is 24/7 service; three 12.5-hour shifts;MAINLY DAYS WITH MINIMAL NIGHTS
(1-2 shifts a month) including weekends, and rotating holidays. Required Criteria Graduate of an accredited PA program Current NYS PA licensure NCCPA certification DEA CertificationBLS/ACLS/PALS
