Full-Time / Part-Time (Flexible Scheduling Available) About the Role We are seeking a compassionate, detail-oriented Physician Assistant (PA-C) to support our growing medical team in skilled nursing facilities and personal care homes. This role focuses on providing high-quality, patient-centered care to older adults and medically complex patients in long-term care settings. The ideal candidate is clinically strong, adaptable, and genuinely enjoys working with geriatric and post-acute populations. Key Responsibilities Conduct comprehensive medical assessments for nursing home and personal care home residents Perform routine follow-ups, acute visits, admissions, and discharge planning Manage chronic conditions and address acute medical issues in collaboration with supervising physicians Order, interpret, and follow up on diagnostic tests and labs Document all patient encounters clearly and accurately in the electronic medical record Communicate effectively with nursing staff, facility leadership, families, and interdisciplinary teams Participate in care plan meetings and contribute to quality improvement initiatives Ensure compliance with facility policies, state regulations, and clinical best practices Qualifications Current Physician Assistant license in the state of practice National certification (NCCPA) Experience in geriatrics, internal medicine, long-term care, or post-acute care preferred (new grads with strong interest encouraged to apply) Strong clinical judgment and time-management skills Excellent communication and interpersonal abilities Ability to work independently while collaborating within a multidisciplinary team What We Offer Competitive compensation Flexible scheduling to support work-life balance Supportive, physician-led clinical environment Opportunity to build long-term patient relationships Meaningful work with underserved and vulnerable populations
