Management of the primary care needs of a panel of assigned pients. Functioning within a Pient Aligned Care Team (PACT), working collaborively with the other PACT members, including the RN Care Coordinor, the LPN, and the Medical Service Assistant. Works with extended PACT members, including pharmacists, mental health professionals, social workers, and dietitians. Providing effective care to pients using sound judgement in assessing, planning, implementing, documenting, and evaluing pient care for assigned pients. Performing history and physical examinions, ordering approprie lab and other tests, evaluing test results, establishing diagnoses, formuling, and implementing trement plans, and modifying trement plans when approprie. Managing prescriptions for assigned pients, including controlled substances if warranted. Working effectively and making approprie requests for assistance from the collaboring physician as approprie. Assisting, as requested, with clinic procedures. Answering pient questions regarding consult placement, follow-up and how to get pients seen. Interacting with consultants as needed for successful referrals. Referencing quality improvement informion and organizes pient care needs to assure safe, efficient, and cost-effective care. Serving as clinic level pient advoce. Acting as the focal point of the designed outpient clinic by being the contact person for all other consultant-specialty services regarding pient care.
