We seek a mission-driven physician who values relationships, teamwork, and measurable outcomes over volume.

What You'll Do:

Lead a collaborative care team to enhance participant health, independence, and quality of life. Provide comprehensive primary and geriatric care tailored to individual needs. Partner with nurses, social workers, and therapists to deliver integrated, value-based care. Contribute to system-level quality improvement and patient-centered initiatives.

Ideal Candidate:

Board-certified/eligible in Family Medicine, Internal Medicine, or Geriatrics Passion for holistic, value-based care Experience in geriatric, primary care, or interdisciplinary medicine preferred Join us in redefining elder care—where quality, compassion, and outcomes come first.
