Diagnostic Radiology with Mammography (biopsy) privileges.

Reading studies of all modalities in addition to diagnostic mammograms with breast biopsies

Credentialing Information:

90-120 days hospital and foundation privileges required. Hospital and Foundation privileges Credentialing with other facilities? No Billing enrollment

Diagnostic Mammograms:

30 They would be asked to read during the day from the list of radiology studies from the emergency department and inpatient. But if there is anything they don t feel comfortable reading, they do have the option to send to MRG which is our Teleradiology group. For the Radiologist in building 4, We mainly need them for mammography and breast biopsies. If they are able to read PET, that would be great appreciated in addition to any other studies (CT, MRI, X-ray, US, etc) Submission Requirements Board Certified in Diagnostic Radiology or Board Eligible

• providers must note in the Additional Submission Details section that they will obtain certification.
• Highly preferred State License CA
• Required Residency Requirements/Fellowship No specific fellowship. Ability to perform diagnostic mammograms and biopsies
• Required BLS/ACLS and DEA during credentialing
• Required Malpractice Clean Highly Preferred Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date.
