Key Responsibilities:

Provide high-quality primary care to adult patients in an outpatient setting Collaborate with an integrated care team including behavioral health, nursing, and case management Maintain accurate and timely documentation in the EMR Participate in quality improvement initiatives and team huddles Advocate for patients and help eliminate barriers to healthcare

Requirements:
