Duties Conduct thorough medical assessments, including physical examinations, sick visits and patient history reviews, to establish accurate diagnoses. Patient ages 18-65 years old. Develop personalized care plans that incorporate preventive strategies, chronic disease management, and health education. Utilize Electronic Health Record (EHR) systems with Elation EMR to document patient encounters accurately and efficiently. Manage acute and chronic conditions across diverse areas or primary care. Coordinate with specialists for referrals as needed. Provide patient service through compassionate communication, health coaching, medication administration, and follow-up care planning. Ensure compliance with HIPAA regulations and maintain meticulous medical records to uphold privacy standards. Assist in clinical research initiatives or quality improvement projects to advance practice standards. Outpatient work only Qualifications Medical degree (MD or DO) from an accredited institution with valid licensure to practice medicine in California. Proven experience in primary care settings with familiarity in outpatient environments. Proficiency with EMR/EHR systems and experience with medical coding (CPT/ICD) is preferred. Strong knowledge of medical terminology, physiology, and clinical evaluation techniques. Experience with telehealth platforms is advantageous. Excellent communication skills with a focus on kindness, empathy and patient-centered care; Ability to work independently while collaborating effectively within a team environment.
