Job Summary:

One of our direct clients is seeking a dedicated Physical Therapist (PT) to join their Home Health team in Kings County, CA/Colorado Springs, CO. The ideal candidate will provide high-quality, patient-centered care, helping individuals restore mobility, improve functional independence, and enhance overall quality of life in a home-based setting.

Key Responsibilities:

Conduct initial evaluations and ongoing assessments in patients homes Develop and implement individualized treatment plans based on physician orders Provide skilled therapy services including therapeutic exercises, gait training, balance training, and manual therapy Educate patients and caregivers on home exercise programs and safety techniques Monitor patient progress and adjust treatment plans as needed Complete accurate and timely documentation in compliance with regulatory standards Collaborate with physicians, nurses, and interdisciplinary team members Participate in case conferences and care coordination

Qualifications:

Active Physical Therapist (PT) license DPT or equivalent Experience (home health preferred) Strong clinical, communication, and organizational skills Ability to work independently in a field-based role

Preferred Qualifications:

Prior home health experience Experience with EMR systems (e.g., Homecare Homebase, WellSky)
