Job Description

  • Family Medicine Physician - Outpatient Primary Care
  • Location:
    Enfield, Connecticut
  • General Description
  • Seeking a Board Certified / Board Eligible Family Medicine Physician for a 100% outpatient primary care setting
  • Full-spectrum Family Medicine providing care for patients of all ages (pediatric, adult, and geriatric)
  • Practice in a mission-driven, team-based environment serving diverse and underserved populations
  • No hospital rounding required
  • Established patient panel with strong community demand
  • Flexible scheduling designed to support work-life balance
  • Integrated care model including behavioral health, dental, pharmacy, and care coordination
  • Advanced Access scheduling model to improve patient flow and access
  • Supported by a robust clinical team including RNs, MAs, and care coordinators
  • Value-based care environment focused on quality outcomes and health equity Key Responsibilities
  • Provide preventive care, annual physicals, and wellness visits
  • Diagnose and manage acute and chronic medical conditions
  • Deliver comprehensive care across the lifespan
  • Coordinate care with behavioral health, dental, and specialty providers
  • Participate in population health and quality improvement initiatives
  • Provide patient-centered, culturally responsive care Qualifications
  • MD or DO from an accredited medical school
  • Completion of an accredited Family Medicine residency program
  • Board Certified or Board Eligible (within 24 months)
  • Eligible for Connecticut medical licensure
  • Bilingual (English/Spanish) preferred
  • Commitment to serving diverse and underserved populations Additional Details
