Skilled Nursing, Inpatient Rehabilitation, and Long-Term CareSeeking a Nurse Practitioner or Physician Assistant to join a post-acute care team in New London, Connecticut. This full-time opportunity involves rounding in skilled nursing, inpatient rehabilitation, and long-term care settings with strong clinical support and earning potential.
Position Highlights:
Full-time positionAverage patient volume of 15-20 patients per dayInpatient rounding across skilled nursing facilities, inpatient rehabilitation facilities, and long-term care hospitalsCollaborative interdisciplinary team environmentWork closely with hospice, palliative care, and home care agenciesWell-developed infrastructure with extensive back-office supportEMR training and support using GehrimedUncapped productivity income potential
Responsibilities:
Provide high-quality, safe, and cost-effective patient care in post-acute care settingsConduct inpatient rounding and manage patient care plansCollaborate with interdisciplinary teams and follow CMS guidelinesEnsure safe transitions of careComplete timely documentation and billing in compliance with Medicare requirements
Qualifications:
Nurse Practitioner or Physician AssistantBoard certified FNP, AGACNP, ACNP, AGPCNP, or PA-CActive Connecticut license or eligibilityWilling to train experienced candidates
