Center of Hope, part of Saint Francis, is seeking a Primary Care

• Advanced Practice Provider to support continued primary care expansion and access initiatives across the Delaware market.

Full-service Family Medicine Center providing primary care to a medically underserved population in the Newark area, with a strong focus on the Latino community. The practice emphasizes cultural component care, supported by a bilingual care team committed to delivering inclusive, patient-centered services. Practice Highlights

• Provide comprehensive primary care for patients across the lifespan
• Assist with the diagnosis and management of acute and chronic conditions
• Emphasize preventive care and population health management
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Maintain timely and accurate documentation
