Position Overview:

We are seeking a dedicated and compassionate Primary Care Physician to join a modern, technology-driven outpatient practice. This role focuses on preventive care, chronic disease management, and coordinated patient care within a collaborative, multi-specialty environment. Physicians will practice with a high degree of autonomy while benefiting from seamless access to specialists for referrals and continuity of care. Key Responsibilities:

Provide comprehensive primary care services to adult patients Diagnose and manage acute and chronic medical conditions Collaborate with specialists and care teams for coordinated treatment plans Manage a patient panel with an average of 16 patients per day Participate in telehealth services (half-day Fridays)

Qualifications:

MD or DO from an accredited medical school Board Certification in Family Medicine or Internal Medicine (or equivalent) Active or eligible Delaware medical license DEA registration

New graduates (no minimum experience required)

Performance-Based Incentives:

20% bonus on ACO revenue 50% share of Chronic Care Management (CCM) fees 50% revenue share after 72 patients/week threshold
