Healthy Families is a nationally accredited, statewide program that is proven to prevent child abuse and neglect before it ever begins. Healthy Families was written into Florida Statute 409.153 in 1998 for the purpose of identifying, funding, supporting, and evaluating programs and community initiatives to improve the development and life outcomes of children and to preserve and strengthen families with a primary emphasis on prevention. Healthy Families provides community-based home visitation services focused on promoting child health and development and positive parent-child interaction. These services include both Parent Education & Support and Mental Health Counseling to participants who are suffering from Postpartum Depression or other Mental Health Issues. Families join the program prenatally or soon after the birth of the baby and can last three to five years, depending upon the needs of the family. Establishes a trusting relationship with the client and family by maintaining regular home visit as required per client level. Maintains dependable, punctual work schedules with a minimum of 80% completed home visits per level requirement. Assists strengthening parent/child interaction by providing in home education, utilizing established program curriculum and by modeling appropriate behavior during each home visit. Travels to participant's homes for regular home visits. Also travels to creative outreach meetings and various agencies for collaboration. Travels to the Healthy Family office for supervision, staff meetings and training. Establishes a family support plan with goals, objectives, and activities with supervisor and client within 90 days of participant agreement. This plan directs every home visit and is reviewed and updated at least every 180 days. Administers Healthy Families America (HFA) tools at the required intervals for each family. Administers Home Safety Checklists with all families, as per schedule, and provides associated teachings as required to include (but not limited to): Shaken Baby, co-sleeping, water hazards, emergency preparedness, and poison prevention. Demonstrates excellent knowledge of community resources, make referrals, and provides follow-up with families to insure needs are met. Maintains appropriate communication with community agencies and providers. Meets with supervisors weekly (1.5 hours minimum) for case management of all families on caseload. Documentation of observations, interactions, and teaching completed at each home visit must be completed and submitted to supervisors within 72 hours of making home visit. All other forms must be completed on time per written protocol. Attends regular staff meeting and case conferences as required. Participates in continuing education training and shares knowledge with other team members. Assists and support ongoing program growth and development. Performs QA audits for families as directed by supervisor to update family data. Performs other duties as required to meet program objectives.NORMAL WORK SCHEDULE
Daily, Monday through Friday, 08:00 AM to 04:30 PM- Total hours per week: 40
