Key Responsibilities:
Provide comprehensive primary care to adult and Medicare Advantage patients Develop long-term patient relationships through a value-based care approach Manage chronic conditions, preventive care, and wellness initiatives Collaborate with a multidisciplinary care team focused on improving patient outcomes
Qualifications:
