, is recruiting a full-time primary care physician for our Ormond Beach, Florida center. We have a unique, value-based care model and we are experts in senior-focused primary care. As a Primary Care Physician, you will report to our Chief Medical Officer/Regional Medical Director. The Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy.

• + Monday-Friday 8:00am-5:00pm + Join an experienced team with an established patient panel + Opportunity to spend more time with patients + Spend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care + Minimal, shared call rotation

• Required Qualifications
• + Doctor of Medicine (MD) or Osteopathic (DO) + Board certified or board eligible in Family Medicine or Internal Medicine - Geriatrics specialty would be ideal + Valid, unrestricted license (or willing to obtain) in the state of desired employment (Florida) + Current, unrestricted DEA license

• Preferred Qualifications:

• + Minimum of two to five years directly applicable experience preferred + New graduates will be considered + Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment + Knowledge of Medicare guidelines and coverage + Knowledge of HEDIS quality indicators + Bilingual (English and Spanish) is a plus •
