WE ARE LOOKING FOR
Franciscan Physician Network is seeking a full-time Nurse Practitioner or Physician Assistant with Palliative Care experience for an Inpatient and Outpatient position to join our team at Franciscan Health Michigan City to meet the growing demands for Hospice and Palliative Care services.
WHAT WE NEED
Ideal candidate will have past Palliative or Home Health Care experience, with a focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.
WHAT YOU CAN EXPECT
Clinic hours: Monday
  • Friday, 8:00 a.m.
  • 4:30 p.
m. Flexible full-time schedule Average outpatient volume: 12 patients per week Average inpatient volume: 6 patients per day Inpatient follow-ups and new consultations Will clarify care planning with patient's family Telehealth offered Certification as a Hospice and Palliative Nurse (ACHPN®) is preferred Eligibility to become certified within 2 years is required
