Job Description

Full-time position in Valparaiso, IN combining clinical care and residency faculty duties. You will provide coordinated, proactive care for patients with chronic conditions and deliver wellness/preventive services.
Schedule:
two days per week in clinic and two days per week teaching and supervising residents in the Physician Residency Program. Compensation and benefits Competitive salary Comprehensive benefits package Professional development and CME support Team-oriented work environment Key duties — clinical Provide outpatient family medicine care, including chronic disease management and preventive services Coordinate patient care and follow-up Perform other clinical duties as assigned Key duties — faculty Teach, supervise, and mentor residents during clinical rotations Assist in arranging resident rotations to meet program objectives Provide academic counseling and participate in resident evaluation Participate in on-site program reviews and ensure compliance with program standards Participate in recruitment and selection of residency candidates Perform additional academic duties as assigned Minimum requirements MD or DO from an accredited medical school Completed residency in family medicine Board certified (or board eligible where applicable) DEA registration CPR certification Indiana medical license (or able to obtain) Active engagement in continuing medical education Valid driver's license and reliable transportation To apply or request more information, please provide CV and references.
