Job Description

Primary Care Provider - Kokomo, IN /
Jeffersonville, IN Job Type:
Full-Time Shifts:
Days (Monday-Friday); Shared on-call coverage (no weekend shift differential)
Healthcare Setting:
Outpatient Senior Care /
Community Health Responsibilities:
  • Provide direct primary care services, including assessments, diagnoses, treatment, and medical record documentation
  • Collaborate with interdisciplinary care teams to coordinate patient-centered care
  • Offer medical consultations and support to clinical staff and care coordinators
  • Participate in care planning meetings and contribute to clinic operations
  • Deliver age-specific and geriatric care with a focus on safety and dignity
  • Ensure compliance with clinical protocols, safety standards, and regulatory guidelines
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Attend staff meetings, trainings, and internal committees as required
Qualifications:
  • MD/DO degree from an accredited medical school
  • Board Certification in Family Medicine or Internal Medicine
  • Licensed or eligible to practice in Indiana
  • CPR certification preferred
  • At least 2 years of experience working with frail or elderly populations in long-term care
  • Experience in senior care or PACE programs is a plus
