Responsibilities:

Perform comprehensive patient assessments Develop and manage individualized care plans Monitor patient progress and coordinate care Collaborate with interdisciplinary care team Educate patients and families on health management Maintain accurate and timely documentation

Qualifications:

Board Eligible or Board Certified in Internal Medicine, Family Medicine, Emergency Medicine, or related field Active Massachusetts license and DEA or ability to obtain Experience in post-acute, acute, ED, or clinic setting preferred

Compensation & Benefits:
