FAMILY PHYSICIAN
Delta Health Center, Inc. Mound Bayou, Mississippi About Delta Health Center Delta Health Center (DHC), located in historic Mound Bayou, MS, is one of the nation's oldest and most celebrated community health centers - established in 1965 as the first Federally Qualified Health Center (FQHC) in the United States. As a cornerstone of the community health center movement, DHC is committed to providing comprehensive, high-quality, affordable preventive and primary healthcare to underserved communities. We proudly serve patients across the Mississippi Delta with compassion, respect, and a deep sense of community responsibility, ensuring equitable access to care regardless of an individual's ability to pay. Position Summary We are seeking a full-time, mission-driven Family Physician to join our clinical team at Delta Health Center. This on-site role is based in Mound Bayou, Mississippi, and involves providing comprehensive primary care services across the full spectrum of family medicine - from pediatrics to geriatrics. The Family Physician will diagnose and treat a broad range of acute and chronic medical conditions, address urgent care needs, and actively support community health initiatives. Collaboration with our interdisciplinary care teams to ensure patient-centered, high-quality outcomes is central to this role. Duties and ResponsibilitiesPrimary Care Delivery:
Provide comprehensive primary care to patients of all ages - pediatric, adult, and geriatric - within the clinic system, with a focus on preventive health, chronic disease management, and health promotion.Diagnosis & Treatment:
Evaluate, diagnose, and manage acute and chronic illnesses including but not limited to hypertension, diabetes, asthma, cardiovascular disease, and infectious conditions, using evidence-based clinical practices.Urgent Care:
Address urgent and walk-in medical needs within the FQHC setting, providing timely assessment and treatment to patients requiring immediate care.Assessment & Examination:
Perform comprehensive patient histories and physical examinations, conduct risk assessments, order and interpret diagnostic and therapeutic tests (e.g., labs, imaging, EKGs), and manage follow-up care accordingly.Prescriptive Authority:
Prescribe medications, therapies, and referrals in accordance with clinic policy, formulary guidelines, and applicable Mississippi State regulations.Chronic Disease Management:
Develop individualized care plans and coordinate ongoing management of patients with complex or multiple chronic conditions, engaging care coordinators and specialists as needed.Preventive Care & Health Education:
Deliver preventive health services including immunizations, screenings, and counseling. Educate patients and families on lifestyle modifications, disease prevention, and wellness practices relevant to the Delta region's health landscape.Community Health Initiatives:
Participate in and support DHC's community health programs, outreach events, and population health efforts targeting underserved and rural patient populations.Interdisciplinary Collaboration:
Work collaboratively within a multidisciplinary team including nurses, mid-level providers, behavioral health staff, and social workers to ensure whole-person, seamless patient care.Specialty Referrals & Consultation:
Recognize conditions requiring specialist intervention and facilitate timely referrals and consultations, ensuring continuity of care and appropriate follow-up.Documentation & Compliance:
Maintain accurate, thorough, and timely electronic health records (EHR) in compliance with FQHC, HIPAA, and clinical documentation standards. Requirements Education and/or ExperienceDegree:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school.Residency:
Completion of an ACGME-accredited Family Medicine residency program is required.Licensure:
Current, valid medical license to practice as a Family Physician in the State of Mississippi, or eligibility to obtain prior to start date.Board Certification:
Board certification or board eligibility in Family Medicine (ABFM or AOBFP) is required.DEA Registration:
Current DEA registration or eligibility to obtain - required.BLS/ACLS
Current BLS certification required; ACLS certification preferred.Experience:
A minimum of one (1) year of post-residency clinical experience in a primary care or outpatient setting is preferred. New residency graduates with strong clinical training are encouraged to apply.FQHC/Underserved Experience:
Prior experience working in a Federally Qualified Health Center, rural health setting, or with underserved populations is a plus. Technical SkillsEHR Proficiency:
Experience with Electronic Health Record (EHR) systems; eClinicalWorks or similar platforms preferred.Software:
Basic proficiency in Microsoft Office applications.Telehealth:

Proven ability to collaborate with multidisciplinary care teams to deliver coordinated, patient-centered care.Bilingual:
