Education/Training Requirements-Must be BC/BE in EM, FP or IM. Must be BC within 5 years of Residency. Clinical Experience Requirements- Recent grads acceptedLicense Requirements- Active unrestricted NC medical license and DEA w/ NC address & schedules 2,2N,3,3N,4 & 5Board Certification Requirements-Will accept
CME/CEU Requirements-CME Attestation form included in the application OR proof of CME documents from the last 2 years.
