Contractual Job Summary Manage and provide the clinical care of patients assigned to the Hospitalist Service through the entire continuum of hospital care while the patient is in the Hospital, including, but not limited to seeing patients in the ER, and following them throughout their hospital stay. Ensure that each patient assigned to the Hospitalist Service receives a medically appropriate level of care throughout the patient's hospitalization and that coverage is provided to patients in the Hospitalist Service on a 24 hour per day/7 day per week basis. Essential Functions Understands and adheres to the WPH Performance Standards, Policies & Behaviors Thoroughly and actively communicate with patient, family and involved parties during scheduled working hours. Coordinate the discharge planning of the patients on the Hospitalist Service with the patient, patient's family and the case manager/discharge planner. Coordinate all aspects of the ancillary services related to discharge including DME, home health, etc. Provide inpatient utilization management for patients in the Hospitalist Service. Consult with specialty physicians and other physicians and provide internal medicine services with respect to patients admitted to the Obstetrical or Surgical service when such consultation is requested. In the event of medical co-morbidities, manage the medical care to be provided to such patients. Provide care in the Critical Care Units in urgent situations. Provide care to a standard which is at all times at least commensurate with that of the medical community of the State of New York, specifically the Society of Hospitalist Medicine, the Joint Commission, the Department of Health and Human Services, New York State Department of Health or any other governmental agency or entity having authority to administer, regulate, accredit or set standards for health care facilities. Report to the Hospitalist Service Directors or the Medical Director or their designees regarding patient management. Collaborate with involved Medical Staff in the management of care. Assist the Hospital in the development of appropriate inpatient, quality assurance, utilization management and cost reduction systems and procedures. Jointly review Hospital operational results with the Hospital on at least a quarterly basis to evaluate the efficacy of Hospitalist cost reduction performance. Join and participate in Medical Staff and Performance Improvement Committees as directed by the Director or his/her designee. Respond to Rapid Response, "Code Blue" and other designated activities within the Hospital. Undertake such community service and committee work, in addition to your clinical responsibilities, as may be requested from time to time by the Director or his/her designee. Provide the highest standards of clinical care and serve as a model colleague to all staff members without discrimination, under any circumstances, toward anyone. Responsibly and accurately submit billing charges within 48 hours of rendering services. Performs all other related duties as assigned Qualifications MD Required or DO Required Residency in Internal Medicine required Required Familiarity with hospital protocols, infection prevention, and patient safety standards Required Strong knowledge of internal medicine and adult disease management (High proficiency) Understanding of acute and chronic medical conditions (High proficiency) Understanding of diagnostic testing, interpretation of labs, imaging, and EKGs (High proficiency) Knowledge of pharmacology, medication management, and adverse drug reactions (High proficiency) Understanding of sepsis, respiratory failure, cardiac events, stroke, and other emergent conditions (High proficiency) Knowledge of discharge planning, transitions of care, and care coordination (High proficiency) Familiarity with regulatory and quality compliance standards (CMS, Joint Commission, HIPAA) (High proficiency) MD Doctor of Medicine
