Radiologic Exam Interpretation (Unofficial) Code stroke response teamRequirement:
Board certified NCCPA•[REQUIRED
] OH License•[ Highly preferred] willing to accept providers needing to obtain license ACLS•[REQUIRED
] A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date.•[REQUIRED
