Salary:
Pay is dependent on experience up to $300k
Yearly Sign On Bonus:
$30k Full health , Dental, Vision, Benefits, PTO provided
Primary Care Physician Specific Duties:
Treat patients 16 and order age group. Develop strong transitional care plans and able to help keep track of paneled patients along the whole continuum of care including discharges from the hospital, ER, skilled nursing facilities/nursing homes, and home care. Treat complex, high-utilization patients with conditions like HIV, diabetes, hypertension, and substance use issues, focusing on "whole-person" care. Educate members on chronic disease progression and the need for advanced directives where applicable
Primary Care Physician Qualifications:
Unrestricted PA Medical License Board Certified in Internal Medicine or Family Practice BLS certification
Preferred Primary Care Physician Qualifications:
