Job Description

A leading value-based healthcare organization is seeking an experienced and mission-driven Lead Physician to provide exceptional care for medically complex and socially vulnerable populations. This role combines direct patient care with clinical leadership, mentoring, and population health management within an innovative care delivery model. About the Organization The organization serves individuals with complex medical, behavioral, and social needs who have often been underserved by traditional healthcare systems. Through an integrated, multidisciplinary care model, it improves health outcomes, enhances quality of life, and reduces unnecessary emergency department visits and hospitalizations. Care teams include primary care providers, nurses, behavioral health specialists, care managers, community health workers, pharmacists, and social support professionals. Services include primary care, infusion therapy, pharmacy, laboratory, imaging, and support addressing social determinants of health such as housing instability, food insecurity, and social isolation. Position Overview The Lead Physician provides comprehensive primary care to a panel of members while serving as a clinical leader within the center. Working closely with the Medical Director, this physician mentors providers, supports interdisciplinary care teams, drives quality and utilization outcomes, and represents the organization within the community.
