This is a hybrid role with the flexibility to work primarily remotely , with occasional in-office responsibilities. This position is ideal for an RN who is detail-oriented, tech-savvy, and comfortable managing patient care in a fast-paced outpatient setting. Responsibilities Manage and triage clinical inbox/messages in a timely manner Respond to patient portal messages and phone calls Coordinate care and escalate urgent clinical issues to providers Review, pend, and track lab orders and results Communicate lab results and follow-up plans to patients Manage and organize incoming documents (labs, consults, imaging) Ensure accurate and timely documentation in the EMR Support patient education on medications and chronic conditions Assist with development and management of a Remote Patient Monitoring (RPM) program Monitor patient-reported data (e.g., CGM, BP, weight) and follow up as needed Requirements Active Registered Nurse (RN) license (New Jersey)

REQUIRED

Experience with eClinicalWorks (eCW)

REQUIRED

Prior experience in endocrinology or primary care Strong clinical judgment and triage skills Excellent communication and organizational skills Comfortable working independently in a remote setting Proficient with EMR systems and patient portals

Pay:

$38.00 - $44.00 per hour
