Job Description
CenterWell Senior Primary Care Primary Care Physician Join a team transforming senior focused primary care. Why You ll Love Working Here Team-based care with strong clinical and operational support Lower daily patient volume for deeper patient relationships Supportive, inclusive culture with opportunities for professional growth Work life balance with generous PTO, minimal call, and dedicated CME time What You ll Do Provide comprehensive outpatient care to adult and senior patients Diagnose and manage complex medical conditions Maintain accurate documentation and coordinate necessary referrals Collaborate with an interdisciplinary care team Participate in quality improvement and clinical initiatives Required Qualifications MD or DO; active, unrestricted medical license Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine Minimum 2 years of experience in value based care or high acuity geriatric populations Commitment to patient centered, outcomes driven care Participation in TB screening Preferred Qualifications Training in Family Medicine, Internal Medicine, Med Peds, or Geriatrics Experience in senior care or value based care settings Proficiency with EHR systems Strong communication and team collaboration skills Experience supervising NPs/PAs Additional Details Full time, outpatient position (Monday Friday, 8 AM 5 PM) Competitive compensation, bonus eligibility, sign on bonus, relocation assistance, and full benefits package Opportunities for leadership development and ongoing professional growth
