Part-Time Nephrologist - Post-Acute Care | San Antonio Region We are a physician-led specialty practice providing Cardiology, Nephrology, and Pulmonology services across skilled nursing and long-term care facilities. As we continue to expand throughout the state, we are seeking a compassionate and collaborative Part-Time Nephrologist to join our growing team. This role offers the opportunity to deliver meaningful specialty care to medically complex patients in the post-acute setting while working alongside experienced Nurse Practitioners and interdisciplinary care teams. Position Highlights Approximately 5 hours per week Flexible scheduling based on your availability Clinical oversight and collaboration with experienced Nurse Practitioners High level of clinical autonomy with strong administrative support Opportunity to make a significant impact in an underserved patient population We provide comprehensive onboarding, ongoing clinical support, and an efficient care model that allows physicians to focus on patient care while maintaining flexibility and work-life balance. Responsibilities Provide nephrology consultation and management for post-acute and long-term care patients Collaborate with and supervise Nurse Practitioners Conduct onsite visits at affiliated skilled nursing facilities Support quality-driven, patient-centered care initiatives Participate in interdisciplinary care planning as needed Qualifications MD or DO from an accredited medical school Board Certified or Board Eligible in Nephrology Completion of an accredited Internal Medicine residency program Active Texas medical license (or eligibility to obtain licensure) Experience managing patients with chronic kidney disease and related conditions preferred Strong communication and collaborative care skills
