Job Details:

Provide personalized care to adult and geriatric patients, including conducting referrals and handling sick visits. 1:1 Provider to

CMA/LPN Ratio:

Work closely with dedicated medical staff to ensure efficient patient care. Conduct E-Visits via MyChart, accommodating patient requests for virtual consultations. Telehealth Visits scheduled based on patient symptoms, transportation needs, or personal preference. Perform Point of Care (POC) testing, including A1C, PT/INR, U/A, Flu, Strep, COVID, Glucose, and Spirometry. Each patient is scheduled for 15-30 minutes, ensuring ample time for comprehensive care. Manage patient call-ins for inquiries (only for providers within the location), with minimal call responsibility (phone calls only). No walk-ins ensuring a structured schedule and minimizing disruptions. About the
