This position is available as either full-time or part-time and supports podiatry and wound care services for medically complex adult and geriatric patients throughout the San Diego service area. The ideal candidate is comfortable practicing independently in the field while maintaining close communication with physicians, Doctor of Podiatric Medicine providers, medical assistants, schedulers, and leadership. Key Responsibilities Conduct comprehensive patient assessments, including wound evaluation and overall health status review Develop and implement individualized treatment plans for patients with acute and chronic wound needs Perform dressing changes, wound management procedures, and other clinically appropriate services within scope Support medication review, pain management coordination, and follow-up care planning within scope Collaborate with physicians, Doctor of Podiatric Medicine providers, facility staff, caregivers, and interdisciplinary teams to coordinate care Educate patients and caregivers regarding wound care management, prevention strategies, and follow-up expectations Monitor patient progress and modify treatment plans as clinically appropriate Complete accurate, timely, and compliant documentation within the electronic health record Travel throughout the San Diego service territory to patient homes and partner facilities Qualifications Active, unrestricted California Nurse Practitioner license Basic Life Support certification required Wound care experience strongly preferred; wound care certification preferred Experience in mobile care, home health, skilled nursing, assisted living, or related field-based settings preferred Strong clinical judgment, communication skills, and documentation abilities Proficiency with electronic medical record and electronic health record systems Valid driver's license and reliable transportation
