Nurse Practitioner - Podiatry & Wound Care ToeTal Health Solutions Las Vegas, NV Job Details Full-time $110,000 - $170,000 a year 1 day ago Qualifications BLS Certification Basic life support Full Job Description Position Overview We are seeking a compassionate and clinically strong Nurse Practitioner to join our Nevada mobile care team. This position is available as either full-time or part-time and supports podiatry and wound care services for medically complex adult and geriatric patients across home and facility-based settings. The ideal candidate is comfortable practicing independently in the field while collaborating closely with physicians, Doctor of Podiatric Medicine providers, medical assistants, facility staff, and administrative teams. Key Responsibilities Conduct comprehensive patient assessments, including wound evaluation and overall health status review Develop and implement individualized treatment plans based on patient presentation, risk factors, and care goals Perform wound debridement, dressing changes, and other clinically appropriate wound care procedures within scope Support pain management, medication review, and care coordination within licensure and clinical protocols Collaborate with physicians, Doctor of Podiatric Medicine providers, nursing staff, caregivers, and facility personnel to ensure continuity of care Educate patients and caregivers regarding wound care, prevention strategies, lifestyle modifications, and follow-up expectations Monitor patient progress and adjust treatment plans as clinically appropriate Complete accurate, timely, and compliant documentation within the electronic health record Travel throughout the assigned Nevada service territory to patient homes and partner facilities Qualifications Active, unrestricted Nevada Nurse Practitioner license Basic Life Support certification required Wound care experience strongly preferred; wound care certification preferred Experience in mobile care, home health, skilled nursing, assisted living, or related field-based settings preferred Strong clinical judgment, communication skills, and documentation abilities Proficiency with electronic medical record and electronic health record systems Valid driver's license and reliable transportation
