As a Podiatrist , you will provide comprehensive foot and ankle care to patients across assisted living communities settings throughout Long Island, Queens, or Brooklyn. This role focuses on delivering high-quality, compassionate, and proactive podiatric care while collaborating closely with facility staff and interdisciplinary care teams. The ideal candidate is patient-centered, reliable, and passionate about improving mobility, comfort, and overall quality of life for post-acute and long-term care patients. Duties & Responsibilities Provide on-site podiatric evaluations, diagnosis, and treatment for patients in assisted living, skilled nursing, and long-term care facilities. Perform routine and advanced podiatric procedures, including nail care, callus and corn management, wound care, ulcer prevention and treatment, and biomechanical assessments. Evaluate and manage foot and ankle conditions related to diabetes, vascular disease, neuropathy, and mobility limitations . Collaborate with interdisciplinary care teams, including nursing staff, primary care providers, and facility leadership, to support comprehensive patient care plans. Maintain accurate, timely, and compliant medical documentation in accordance with organizational standards and regulatory requirements. Educate patients, caregivers, and facility staff on foot health, preventive care, and treatment plans. Ensure compliance with all state, federal, and facility-specific healthcare regulations and standards. Support continuity of care by coordinating follow-ups, referrals, and ongoing treatment plans as needed. Job Types:
