Job Description

Segue Inc. is a non-profit outpatient mental health facility that has been providing quality mental health services to the citizens of Jackson & Hillsdale counties in Michigan for over 30 years, offering intensive and essential services and programs. We are searching for a qualified Team Leader for our jackson, MI, facility.
POSITION SUMMARY
The Team Leader leads the team in providing Assertive Community Treatment (ACT) services to individuals with chronic mental illness who require intensive outpatient treatment. This individual gives clinical guidance, direction, and support to their team and the Impact program's consumers. The Team Leader is responsible for administering the ACT outpatient treatment program following policies and procedures, Medicaid, and CARF standards. Considerable independent judgment is used in carrying out the duties.
QUALIFICATIONS
Master's degree in Psychology, Social Work, or related human services field Training and knowledge of mental health and community resources with two years of direct service experience Licensure in social work or psychology in the State of Michigan is required Certification in addiction through MCBAP is required Able to type 35 wpm with a high degree of accuracy; experience working with EMR preferred Knowledge of the Mental Health Code, Medicaid Standards, and CARF standards is required Must live within a 45-minute radius of the Hillsdale office to meet the on-call responsibilities
ESSENTIAL JOB RESPONSIBILITIES
Provides leadership oversight to the ACT program; leads daily team meetings and coordinates activities. Offers clinical supervision and guidance to the team; directs appropriate intervention strategies; takes the lead in crises. May lead group sessions for consumers. You will provide therapy to our ACT consumers as needed. Directs monitoring of consumers released from the hospital for 30 days with plans to reduce recidivism. Responsible for treatment planning meetings, developing the treatment plan, and quarterly treatment monitoring reviews. Counsels clients individually, in family, or other small groups. Conducts 60-day new-hire and annual performance appraisals and participates in staff disciplinary actions. Monitors program activities to ensure compliance with applicable laws, regulations, CARF, and contract provisions. Keeps the team and administration informed of important client or staff information. Works closely with local service providers to coordinate the delivery of services, including hospitals, courts, and law enforcement agencies. Represents Segue in the community, including other public and private agencies. Conducts interviews to fill vacancies in the Impact program. Responsible for the orientation of new employees. Responsible for counseling and disciplining staff. Participates in the termination of employees. Gathers statistical information for QI and is a Professional Staff Organization (PSO) member. Arranges for and presents cases to Behavioral Risk, PSO, Court/Judge, or other bodies. Is a consultant for after-hours on-call staff regarding hospitalization and/or crisis home placement. Attends clinical supervision with the Clinical Director and as required to maintain licensure. Required to work additional hours or other duties as directed by Administration.
